What does cyber-insurance actually require now?

Typically multi-factor authentication everywhere, endpoint detection and response, tested offsite backups, and written evidence of all three. Attesting to controls you do not have can void the policy.

Is backup really separate from Microsoft 365?

Yes, and this catches people out. Microsoft replicates your data for their own resilience; they do not keep a long-term restorable copy for you. Retention after deletion is limited.
